OMMA MED PORTAL ISSUE INTAKE

Given the high volume of licensees experiencing difficulties with the OMMA Med Portal and extended wait times when contacting the OMMA call center, we've developed a simple form to address common portal issues more efficiently. This form will connect your information directly with OMMA's licensing department to expedite resolution of your concerns.

INSTRUCTIONS: Answer all questions truthfully and with as much detail as possible.
